Spinal cord stimulation (SCS) helps manage chronic pain by sending gentle electrical signals to the spinal cord, interrupting pain signals before they reach the brain. Two main approaches exist: percutaneous and paddle lead.

The percutaneous approach involves threading small wires through a needle without extensive surgery. This method is less invasive, often performed by pain specialists, and allows for quicker recovery. However, it may not be feasible for patients who have undergone previous spinal surgeries or have hardware that may impede the approach, as these can obstruct access to targeted areas.

The paddle lead method, however, may be more stable and effective in certain complex cases. Performed by spine surgeons, this technique involves a laminectomy, where part of the vertebral bone is removed to strategically place wider paddle leads near the spine. This approach ensures better stability, making it suitable for patients with challenging anatomy from prior surgeries.

Candidates for SCS may include those with chronic back or leg pain unresponsive to other treatments, those with complex regional pain syndrome, painful diabetic neuropathy of the feet, and others that have failed previous back surgery. There are other indications as well, so make sure you check with your provider to see if you qualify. Alternatives include pain medications, physical therapy, and injections.

All surgical procedures carry risks, including potential infections and, rarely, nerve damage or epidural complications. Detailed pre-surgical preparation and adherence to post-operative guidelines can maximize the chance of a good recovery.
