Hip and Greater Trochanter Bursa Embolization (HAE)

Hip and greater trochanter bursa embolization, or HAE, is a minimally invasive procedure designed to help relieve chronic hip pain by targeting inflammation in the tissues around the hip. This type of pain often occurs over the greater trochanter, a bony area on the side of the hip, and may result from long-term bursitis or tendon irritation that doesn’t improve with other treatments like physical therapy, injections, or regenerative therapies such as PRP (platelet-rich plasma). Over time, these issues might lead to the development of abnormal blood vessels that sustain inflammation and can carry pain signals. HAE focuses on addressing these troublesome vessels to reduce inflammation and alleviate pain.

Using imaging guidance, like live X-ray, the procedure delivers precise treatment without incisions. A small tube, or catheter, is inserted into an artery, typically through the foot, much like placing an IV. A special dye highlights the abnormal vessels. Tiny beads are used to block these vessels, decreasing inflammation and calming nearby pain nerves. These beads aren’t felt and stay permanently in place.

The procedure generally lasts 30 to 60 minutes, with patients usually going home the same day with a small bandage. Pain relief is progressive, with most patients reporting improvement around two weeks after the procedure, continuing over the following weeks. About 70% of patients experience significant pain relief, meaning at least a 50% reduction in pain, enhancing daily activities.

While HAE is a promising option for chronic hip pain, alternatives include continued physical therapy, medications, further injections, and regenerative treatments.
